MEDICAL RELEASE & ACKNOWLEDGEMENT

At Light & Joy Studio, your child’s safety and well-being are our top priorities. We take every precaution to provide a safe and caring environment for all participants in our programs, camps, and events.

In the unlikely event of a medical emergency, parent(s) and/or guardian(s) authorize qualified and licensed medical professionals to diagnose and provide treatment for their child when, in the opinion of the attending medical professional, immediate care is needed to prevent serious harm, pain, or further injury.

This authorization includes permission for necessary medical or minor surgical treatment, x-ray examinations, and immunizations as determined appropriate by the attending medical professional.

Every effort will be made to contact the parent(s)/guardian(s) as quickly as possible in the event of a serious illness, major injury, or emergency requiring significant medical care. This authorization is valid only after reasonable attempts have been made to reach the parent(s)/guardian(s).

Parent(s)/guardian(s) also grant permission to Light & Joy Studio staff and its affiliates to provide necessary first aid or emergency support prior to a child’s admission to a medical facility, if needed.

This medical release applies only during the dates of the enrolled camp, program, or event.

By completing registration, parent(s)/guardian(s) acknowledge their understanding and acceptance of this policy, which authorizes medical treatment under emergency circumstances to protect the health and safety of their child. Light & Joy Studio is committed to responding promptly, compassionately, and responsibly in all emergency situations.
